The Mariveles Water District through its Bids and Awards Committee (BAC) invites contractors registered with and classified by the Philippine Contractors Accreditation Board ( PCAB) to apply for eligibility and if found eligible, to bid for the hereunder contract:”

We would like to request you to please quote your lowest price/s for the item/s listed in the attached form “Request for Proposal Form (RPF)”. Your submission shall be prepared in accordance with the following:
Terms and Conditions:
1. Indicate or write your quotation using the attached form in the Bidding Documents “Bill of Quantites Form (BOQF)” and duly signed by your authorized representative. Supplemental information, if any, using the company stationery may be attached to reflect the complete specification of the bid e.g., brand name, model, pictures/brochures/literature, etc.
2. Any interlineations, erasures, or overwriting shall be valid only if they are signed or initialed by the bidder or his duly authorized representative/s.
3. Award of contract shall be made to the Most Economical, Advantageous and Responsive Bid, which complies with the minimum item/specification as stated in BOQF and other terms and conditions stated in the Bill of Quantity Form.
4. Subject for submission of warranty certificate/Performance once awarded, whenever necessary.
5. All proposals can be submitted through the following means: a) hand carried/thru LBC SEALED ENVELOPE, should be properly labeled and signed, or b) thru manual submission in Mariwad Office.
6. Quotations should not exceed the Approved Budget for the Contract (ABC).
7. Alternative quotations or quotations with multiple options shall be rejected.
8. Infrastructure project shall be delivered to Pump Stations Locations.
Eligibility and Other Requirements:

Prospective bidders should possess a valid PCAB License applicable to the contract, have completed a similar contract with a value of at lease 50% of the ABC, and
have key personnel and equipment (listed in the Eligibility Forms) available for the prosecution of the contract. The BAC will use non-discretionary pass/fail criteria
in the Eligibility Check/Screening as well as the Preliminary Examination of Bids. The BAC will conduct post-qualification of the lowest calculated bid.
All particulars relative to Eligibility Statement and Screening, Bid Security, Performance Security, Pre-Bidding Conference, Evaluation of Bid, Post-Qualification and Award of Contract shall be governed by the pertinent provisions of R.A. 12009 and its Implementing Rules and Regulation (IRR).
Note:
1. With the end view of obtaining the contract most advantageous to the government, BAC reserves the right to: a) accept or reject any or all the quotations and waived any minor deviation in the quotation which will not materially affect the substance of the bid; b) award the contract on a per project/lot basis; and c) annul the procurement process and to reject all quotations at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.
2. All bidders will be informed of the results of this procurement.
3. Failure to comply with this delivery schedule shall be subject to Section 71.1.4 (Liquidated Damages) of the IRR of R.A. 12009.
