1. The Philippine Ports Authority, Port Management Office of Bicol, through its Bids and Awards Committee invites interested constructors to submit quotation for the hereunder project:

4. Latest Income and Business Tax Returns
5. Notarized Omnibus Sworn Statement
6. Bid Forms/Bill of Quantities, and
7. Detailed Estimates
8. List and Qualification of Key Personnel to be assigned to the Contract/Project
(Attached individual resume and PRC License of the professional personnel)
9. List of equipment as specified above with attached proof whether owned or lease
and/or under Purchase Agreements Pledged to the proposed contract
Non-submission of the above required documents shall be ground for disqualification.
3. Quotations shall be submitted in person to Mr. Ferdinand C. Urmaza, BAC Secretariat, Philippine Ports Authority, Port Management Office of Bicol, Regional Government Center, Rawis, Legazpi City in a sealed envelope. Quotations/documents sent to any other persons/personnel and those that are not sealed will not be considered and accepted.
