Please quote your best offer for the item/s described in the herewith Price Quotation Form (PQF), subject to the Terms and Conditions provided in this Request for Quotation (RFQ). Submit your quotation duly signed by your authorized representative not later than the date and time of opening of quotation indicated above at the Bids and Awards Committee Office, Ground Floor, Main Building, Capitol Compound, Brgy. Payompon, Mamburao, Occidental Mindoro.
A copy of your updated Business/Mayor's Permit and proof of PhilGEPS Registration are also required to be submitted along with your signed quotation/proposal. Omnibus Sworn Statement and Income/Business Tax Return will also be required for Projects with ABCs of above Php 500k.

TERMS AND CONDITIONS
(1) For procurement amounting to Php 50,000.00 and above, prospective suppliers are required to pay a non-refundable amount of Five Hundred Pesos (Php 500.00) to the cashier of the Provincial Treasurer's Office.
(2) Price Quotation must be valid for the period of thirty (30) calendar days from the start of submission.
(3) Award of Contract shall be made to the lowest quotation per lot, which complies with the minimum technical specifications and other terms and conditions stated herein.
(4) Any interlineations, erasures or overwriting shall only be valid if they are signed or has the initial of the supplier or his/her duly authorized representative(s).
(5) Liquidated damages equivalent to one tenth of one (0.001%) of the value of the goods not delivered within prescribed delivery period shall be imposed per day of delay. The BAC of Province of Occidental Mindoro shall rescind the contract once the cummulative amount of liquidated damages reaches ten percent (10%) of the amount of the contract, without prejudice to the other courses of action and remedies open to it.
(6) The Bids & Awards Committee of the Provincial Government of Occidental Mindoro reserves the right to accept or reject any or all quotations and to impose additional terms and condition as it may deem proper.
(7) In case of failed canvass due to insufficient number of complying offers, a re-canvass maybe conducted if there is still adequate time to do so without jeopardizing the urgent need/delivery of these particular materials/supplies.
(8) Quotation(s) above the Approved Budget Cost will be deemed unacceptable.
