Supply, Delivery, Installation, Testing and Commissioning of Fire Detection and Alarm System, Per Technical Specifications

Please refer to the attached Request for Quotation Form and Terms of Reference for Details and Other Conditions.

1. All entries in the proposal/quotation must be typewritten.
2. Price quotation must be based on the Terms of Reference (TOR) / Scope of Work/Services (SOW) / Technical Specifications (TS), as applicable and must be duly signed by the supplier’s authorized representative.
3. Price quotation (Unit Cost) must be inclusive of VAT and other applicable taxes/charges.
4. The price quotation must include the documentary requirements (see below) and must be submitted through the email address below or to the Procurement Unit-PIMD 6th Floor, DBP Head Office, Sen. Gil Puyat Ave., cor. Makati Ave., Makati City.
5. No down-payment or advanced payment.
6. Deadline of submission is not later than 19 October 2026 (05:00 PM).

Documentary Requirements:
1. Signed Formal Quotation/Proposal (using Supplier's Letterhead) with Brand and model /Scope of Services/Technical Specifications to prove compliance with DBP requirements;
2. Proof of PhilGEPS Registration;
3. Valid/Updated (2026) Mayor’s/Business permit;
4. NOTARIZED Omnibus Sworn Statement [with eleven (11) statements];
5. NOTARIZED Secretary’s Certificate/SPA, as applicable (for suppliers under Partnership and Corporation/Sole/Single Proprietorship);
6. Latest Income Tax Return
7. Signed/Conformed DBP Terms of Reference (with date, name and signature of Supplier’s Authorized Representative per page);
8. Accomplished and Signed DBP Privacy Consent Form (with date, name and signature of Supplier’s Authorized Representative); and
9. Certificate of Site Inspection to be issued by respective DBP Branches.
