1. The Philippine Ports Authority, Port Management Office of Bicol, through its Bids and Awards Committee invites interested constructors to submit quotation for the hereunder project:

H. Approved Budget for the Contract : Php 290,278.84

Equipment Required: 1 Unit – Dump Truck (6.89 – 9.17 cu.m); 1 Unit – One bagger concrete mixer; 1 Unit – Concrete Vibrator (3.5 hp) ; 1 Unit – Jack Hammer (U/W); 1 Unit – Concrete Cutter; 1 Unit - Air Compressor (250 cfm); 1 Unit - Plate Compactor; 1 Unit - Electric Bar Bender (25mm dia) ; 1 Unit - Bar Cutter (25mm dia.)
2. Bidders shall submit their valid and current eligibility requirements properly book-bound detailed as follows:
Documentary Requirements:
1. Valid and Current Mayor’s/ Business Permit
2. Philgeps Registration Number
3. PCAB License (at least Small A for Building and Industrial Plant & Electrical Works)
4. Latest Income and Business Tax Returns
5. Notarized Omnibus Sworn Statement
6. Bid Forms/Bill of Quantities, and
7. Detailed Estimates
8. List and Qualification of Key Personnel to be assigned to the Contract/Project
(Attached individual resume and PRC License of the professional personnel
9. List of equipment as specified above with attached proof whether owned or lease
and/or under Purchase Agreements Pledged to the proposed project.
Non-submission of the above required documents shall be ground for disqualification.
3. Quotations shall be submitted in person to Mr. Ferdinand C. Urmaza, BAC Secretariat, Philippine Ports Authority, Port Management Office of Bicol, Regional Government Center, Rawis, Legazpi City in a sealed envelope. Quotations/documents sent to any other persons/personnel and those that are not sealed will not be considered and accepted. Quotations may be also submitted thru courier to the given address or thru email to pmpbicolbac@gmail.com in PDF format.
